Education

Educate residents, families and partners about cultural differences

To keep up to date on medicare and other managed care insurance

Educate residents,families and partners about advanced directives
Clinical

Participate and provide therapeutic intervention to assist resident's in coping with their transition and adjustment to their stay at NHC

Conduct ongoing psychosocial interviews on each resident

Provide as needed counseling to residents and their families
Advocacy

Promote and protect residents rights

Promote the dignity and worth of each resident

Build upon each resident's strengths and self esteem
Discharge Planning

Coordinate each resident's discharge planning process

Collaborate with NHC inter-disciplinary team members to develop individual patient discharge plans
